The importance of medical records in the informed consent form impacted to the quality of the form, because the form will be used as one of the valid evidence base if there is an accusation in court. The author conducted a preliminary survey in hospitals Semarang, from ten (10) document medical records of surgical cases, there were 7 who not filling complate. This study aims to Know the contents, the implementation of filling, as well as the completeness of the patient's informed consent form sectio caesarea based on aspects of medical records.This is a descriptive research method with cross sectional approachment. Data was collected by observation, in-depth interview and checklist. Data proccesing method by collecting editing and tabulating. Data was prsenting by sentence and paragraph.The result of the research shows the informed consent is not appropriatewith the medical record aspect, because the content of completeness aspect is not at 100% but only reached 87.5% to complete the form consent medicine and 78.6% for anesthesia consent form , In addition, the large number of patients becoming obstacles in the completeness of consent medicine in patients sectio caesarea. So, it should be improved, especially on completeness column number of medical record and authentication doctor for identification patient an to find out who is responsible for services thathas been given to the patient.
