Abstract

Outpatient medical records are the title of an outpatient form in the gayamsari health center and there is one form. PERMENKES No.269 / 2008 article 5, paragraph 2 states that medical records should be made promptly and completed after the patient receives the service. Preliminary study results at Gayamsari Health Center 100% incomplete. After each service, the counter attendant performs daily checks. But there are still incomplete medical records. The purpose of this research is to know the completeness of outpatient medical record at Gayamsari Health Center. The type of research is quantitative descriptive. The study population is an outpatient medical record in 2017 at Gayamsari Health Center. The sampling technique is Random Sampling by lottery (arisan). Sample size was calculated using slovin formula with error rate of 10%, obtained 100 samples. Data collection method is observation using checklist. The results showed that an incomplete patient identification review was 88%. The most incomplete component items are allergies. An important review report shows that 92% is incomplete. The most incomplete component items are U (Age) and BB (weight). An authentication review shows that it is incomplete at 17%. The most incomplete component item is the name. The listing review shows that incomplete of 29%. The most incomplete component items are the blanks.
