dc.descriptionEarly Warning Alert and Response System (EWARS) is a system which has capability to early detect on infectious outbreaks disease threat. In Fact, EWARS report does not run properly, so it is too late to be treated by the local medical instance. EWARS data of East Java on 6th week in 2017 shows that the completeness and accuracy of reports still does not run optimal. Surabaya only has 35% of EWARS on-time report; it is way from target of 80%. Meanwhile, Surabaya has 37% of completeness of EWARS report on that week. Objective study for modelling design tools report supporting EWARS in public health center (PUSKESMAS) level of Surabaya. Research was using descriptive plan to aim for describing and designing supporting tools for EWARS in public health center (PUSKESMAS) level of Surabaya. Data collection was done by deeply interviewing to the person in charge of EWARS of Surabaya health office and person in charge of EWARS at Puskesmas level by purposive sampling. Result by the interview, we acquired information in regard to output of planning of supporting tools for EWARS report. Meanwhile, we also conducted the identification process to produce expected information. Then, we also conducted the identification of input component in planning the supporting tools. The supporting tools for EWARS report in puskesmas level will produce EWARS report per day. The supporting tool was made by computerizes-based. So it can minimize miscalculation. In use, it provides tally column to help the process of data recording of EWARS per day. Conclusion The research produces alternative solution in EWARS report at Puskesmas level, which is the planning of supporting tools will produce information of EWARS per day. It needs to standardize form of EWARS recording and reporting data at Puskesmas level. ABSTRAK Sistem Kewaspadaan Dini dan Respons (SKDR) merupakan sebuah sistem yang memiliki kemampuan untuk melakukan deteksi dini terhadap ancaman KLB penyakit menular. Fakta di lapangan, pelaporan SKDR tidak berjalan dengan semestinya, sehingga KLB terlambat diketahui oleh instansi kesehatan yang berwenang. Data SKDR di Provinsi Jawa Timur pada minggu ke-6 tahun 2017 menunjukkan bahwa kelengkapan dan ketepatan laporan SKDR masih belum optimal. Kota Surabaya hanya 35% laporan SKDR yang tepat waktu, sangat jauh dari target sebesar 80% laporan tepat waktu. Selain itu Kota Surabaya hanya memiliki 37% dalam kelengkapan laporan SKDR pada minggu tersebut. Tujuan penelitian untuk merancang model desain alat bantu penunjang laporan sistem kewaspadaan dini dan respons di tingkat puskesmas Kota Surabaya. Penelitian ini menggunakan rancangan deskriptif yang bertujuan untuk menggambarkan dan menyusun desain alat bantu penunjang laporan sistem kewaspadaan dini dan respons di tingkat puskesmas Kota Surabaya. Pengumpulan data dilakukan dengan wawancara mendalam kepada penanggung jawab sistem kewaspadaan dini dan respons di Dinas Kesehatan Kota Surabaya dan staf penanggung jawab sistem kewaspadaan dini dan respons di puskesmas secara purposive sampling. Hasil dari hasil wawancara mendalam dengan penanggung jawab SKDR didapatkan informasi terkait output yang ingin dihasilkan dari perancangan alat bantu penunjang laporan SKDR. Selain itu, juga dilakukan identifikasi proses yang perlu dilakukan untuk menghasilkan informasi yang diinginkan. Kemudian juga dilakukan identifikasi pada komponen input dalam merencanakan rancangan alat bantu tersebut. Alat bantu penunjang laporan sistem kewaspadaan dini dan respons tingkat puskesmas akan menghasilkan informasi data SKDR per hari. Alat bantu tersebut dibuat secara komputerisasi sehingga dapat meminimalisir kesalahan perhitungan. Dalam pengisiannya, disediakan kolom Tally untuk mempermudah dalam proses pencatatan data SKDR per hari. Penelitian ini menghasilkan solusi alternatif dalam pelaporan SKDR di tingkat puskesmas, di mana rancangan desain alat bantu penunjang laporan SKDR akan menghasilkan informasi data SKDR per hari. Perlu dilakukan standarisasi form pencatatan dan pelaporan data SKDR di tingkat puskesmas.  en-US
