Leptospirosis is zoonotic diseases which is a health problem in Gresik District. CaseFatality Rate (CFR) of leptospirosis in 2009 : 38,13%, in 2010 : 33,33 % and increased45% in 2011. Leptospirosis case in 2011 as much 20 cases and in 2012 increased to 26 cases. The aim of this research is to determine of vulnerable zone of leptospirosis in Gresik District. This researching done from March until November 2012, using spatial analysis and cross sectional design.Result : in 2012 leptospirosis cases as much 26 (14 clinic cases and 12 confirm cases) which dominant by adult males as fisherman and farmer. Distribution of leptospirosis consist of3 clusters : Duduk Sampeyan, Bungah and Gresik.Leptospirosis case increasing in January until April according rainfall patern. Land use for fishpond and farming, medium rainfall, lowland, medium vegetation, alluvial soil and existence of rats are environment variable that influence of leptospirosis case. High vulnerable zone on center's Gresik district.
